No paperwork is required to sign up for the Spiff Program. Simply complete the secure form below to enroll. After the form is complete, two key steps will occur: 
  1. You will receive a Dauphin Spiff card in the mail that needs to be activated for your funds to be added.
  2. You will receive an email from a Dauphin representative with your unique 6-digit Spiff card number and a 4- digit Spiff code  

When you place an order eligible for a spiff, it must include your 4-digit Spiff code on the order. After the order ships, the funds will be added to your card within 6 weeks.
